Your Clinical Care Pathway
We assess symptoms, weight, blood pressure and fluid status, review heart and kidney medicines and arrange blood tests.
Echocardiography or cardiology review may help guide decisions.
The plan includes monitoring after medication changes.
Expected benefits and limitations
Care aims to ease congestion and breathlessness while maintaining effective heart treatment and monitoring kidney safety. A change in creatinine does not always mean that helpful heart medicines should be stopped.
